Eligibility
- Degree/Diploma in Pharmacy.
- Minimum 1 year of experience in Managing a drug store in a reputed hospital/ health center recognized by the Govt
Age: 21 yrs–32yrs as on 1.12.2015
Monthly Remuneration :13,800/-+ PI
Selection Criteria
Selection will be made based on the assessment of their academic career as follows.
- Matriculation: 15 marks
-+2 Science: 15 marks
-D.Pharma/B.Pharma: 70 marks

N.B–
A candidate obtained Diploma/Degree in Pharmacy will liable for assessing the marks in the final exam. Irrespective of Diploma/Degree. For example, a candidate having degree qualification will be deprived of the marks secured in Diploma course for assessment.
GENERAL INFORMATION AND INSTRUCTIONS
1. The above positions are purely temporary and co - terminus with the scheme. Canvassing in any form will render the candidate disqualified for the position.

3. Candidates are required to apply in prescribed format availble in the website www. nuapada.nic.in and to submit along with a set of self attested copies of all certificates in support of age, qualification, experience, residence etc.Candidates are also required to submit two recent passport size colour photograph, an ID proof (Voter ID/PAN/Driving Licence/Passport/Adhar card) and recent (Issued within 6 months as on 1.12.2015) Caste & Residence certificate issued by the competent authority. No certificate will be accepted at later stage of recruitment, if fails to produce with the application.
4. Candidates, who are already working in Health Department or outside either on regular or on contractual basis, have to submit No Objection Certificate from concerned employer.
5. For all the post candidates must have passed Odia up to M.E Standard.
6. Interested candidates fulfilling the eligibility criteria are to apply to the “Chief District Medical Officer cum District Mission Director, Nuapada, District - Nuapada (Odisha) PIN-766105” so as to reach on or before 21.12.2015 (by 5.00 P.M.) through Regd.Post / Speed Post / Courier only and the envelope containing the application should be superscribed clearly name of the post applied for .
7. The applications received for all the posts will be scrutinized and the database will be uploaded in the above website for invitation of objection. Candidates are required to visit website:- www. nuapada.nic.in at regular intervals for any notification, updates (objection invitation / different tests notices), results etc. relating to recruitment.
8. Candidates will be selected on the basis of selection process as applicable. Only shortlisted candidates will be called for participation in the selection process.
9. Incomplete application in any form will be rejected.
10. If any candidate is found to have suppressed any material information or furnished false information /documents, his/her case shall not be considered for the post applied for and in case already engaged on the basis of the said information/documents, his/her service shall be terminated from the Society forthwith. Candidates who have been disengaged earlier from the OSH&FW Society on administrative ground such as disobedience/ poor performances/ misbehavior/ criminal activity etc. are not eligible to apply.
11. This office will not be held responsible for any postal delay. No application will be entertained after 21.12. 2015 (5.00 P.M.). No personal correspondence/queries will be entertained. All communication will be made through email / district website.
12. The panel for above positions shall also remain valid for similar post/s in other programmes under ZSS ambit with same educational qualification and same remuneration, as will be decided by the Society. 13. Number of vacancies / remuneration as mentioned under this advertisement may vary at the time of actual engagement.
14. The post of Staff Nurse (SNCU, NBSU, DP, KMC, NCD) & LT (DP/BB/Sickle cell) will be filled up from the common panel, positioning of the selected candidates will be made on merit-cum-option basis.
