Job Description
The Pharmacist 3 will be responsible for providing clinical pharmacy services in General Oncology at the Seattle Cancer Care Alliance (SCCA)/University of Washington Medical Center. Implement and monitor, with the health care team, patient medication therapy for achievement of desired therapeutic outcomes. Maintain accurate medication profiles for patients. Utilize the system of Continuous Quality Improvement (CQI) to measure the effectiveness of pharmaceutical care. Participate in protocol development. Participate in the education and training of health care students and providers. Perform the duties of a Pharmacist 2. Maintain patient confidentiality according to UW Medical Center standards. Perform related duties as required.
Candidate Profile
• Pharmacy degree (eligibility for State of Washington Pharmacist License) AND two years of experience in pharmacy practice.
• Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
• Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
