Job Description
1. Conducting pharmaceutical care to the patients suffering from chronic disease
2. Providing safe and appropriate guidelines on use of medications, and be an advocate of rational drug therapy through the following:
• Evaluation of the appropriateness of drug therapy based on patient specific factors
• Individualizing of drug therapy
• Evaluation medical conditions and providing drug information
3. Routinely monitoring the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities
4. Communication with physicians, nurses, and other healthcare providers as necessary
5. Conducting health camps and counseling the patients across the communities.
6. To work with pharmacy team to provide comprehensive clinical pharmacy service to patients to enhance patient safety and ensure medication management practices are safe and effective
7. Providing guidance for disease specific diet and lifestyle

Candidate Profile
Fresher with excellent communication skills and the passion to serve the patients with empathy
Candidates with fluent English, hindi and telugu linguistic skills will be preferred.
