Applications are invited for the following posts in the prescribed format/ Performa given below for selection and engagement on contractual basis under the establishment of C.D.M.O., Jharsuguda through Regd. Post/ Speed Post only., so as to reach the Office of the C.D.M.O., Jharsuguda, P.O./ District-Jharsuguda within 10 (ten)' days of publication of advertisement in News Paper. Incomplete application/ applications received after due date shall not be considered under any circumstances.
Post: Pharmacist (DDC)
No.of post: 13 (UR-7, ST-3, SC-2, SEBC-1)
Remuneration: Rs. 5200/- P.M.
Essential Qualification: Must have passed D.Pharma Examination from any of the three Medical Colleges / Odisha Govt, recognized Institutions approved by A.I.C.T.E. only and have registered their names in Orissa Pharmacy Council.
AGE: Minimum 21 years and maximum 32 years as on date of advertisement. Upper age relaxation as per recent govt, rules.
MODE OF SELECTION: The selection w ill be made strictly on the basis of carrier marks as per as per recent govt. rule.
GENERAL CONDITION:
1. The engagement is purely on contractual basis & temporary and may be terminated at any time without notice and without assigning any reason thereof.
2. Candidates must have registered their names in Employment Exchange of Jharsuguda District and have Valid Registration Card.
3. Number of Posts under different category may vary as per norm / vacancy position at the time of selection.
4. The candidates cannot claim any regular appointment in future as a matter of right and also will not claim inter-district transfer. They will not claim any financial benefit other than the monthly consolidated remuneration and other allowances as per State Govt. Rules.
5. They will not claim any Govt, accommodation.
6. Preference will be given to the candidates of Jharsuguda District (having nativity / residence certificate for service only), in case sufficient candidate will not be available in Jharsuguda District, applicants belonging to other district will be considered.
7. O.R.V. Act will be applicable as per Govt. Rules prevailing at the time of selection.
8. The undersigned reserves the right for cancellation / modification of the Advertisement and mode of selection (as will be decided by the selection committee and as will be instructed by the Govt.) at any time without assigning any reason thereof.
9. The in-service candidate should apply through their employer or have to submit No objection certificate for the employer.
10. They will apply to the Chief District Medical Officer, Jharsuguda by Regd. Post/Speed Post only. The Office will not be responsible for any postal delay.

DOCUMENTS TO BE SUBMITTED WITH THE APPLICATION
a. Attested copies of Educational Certificates and Marks Sheet from H.S.C. Examination onwards along with the proof of Date of Birth.
b. Attested copies of valid Caste Certificate in case of SC/ST/SEBC obtained from the competent authority.
c. Attested copies of valid Nativity Certificate issued from competent authority for the purpose of service only.
d. Three recent Passport size Photograph duly attested by Gazetted Officer.
e. Attested copy of valid Registration Certificate from concerned Council.
f. Two Nos. of self addressed Registered Post Envelope (24x1 Ocm) duly stamped to be enclosed with the application.
g. No objection certificate from the current employer if employed.
h. Incomplete applications and those without all of the above required documents will be summarily rejected.
The Envelope containing the application form should be superscribed as “APPLICATION FOR THE POST OF (NAME OF THE POST) in block capital letters and the candidates should mention their respective categories on the envelope (i.e. tJR/SC/ST/SEBC).
