Out of UR/ Gen. Category 08 Nos. of Pharmacist Post, one person is Blindness or Low vision and out of 03 Nos. of ST Category Pharmacist Post, one Person is Ex- Service Man.
Monthly Remuneration: Rs. 5,200/- and G.PRs. 2,400/- Per Month
Minimum Qualification: Must have passed HSC and + 2 Science Examination under Council of Higher Secondary Education, Odisha / equivalent and Diploma in Pharmacy from any of the 3 (three) Medical College & Hospitals of the State / any other recognized private institutions duly approved by A.I.C.T.E. and examination conducted by the Odisha Pharmacy Board.
Selection: Selection will be made on the basis, as per the Career Assessment Mark in HSC Examination excluding 4th Optional and + 2 Science Examination excluding 4th Optional and Diploma in Pharmacy Examination. The Pharmacist concerned must have registered their name in Pharmacy Council in the state and have possessed valid Registration Certificate as on the date of Advertisement
Document to be submitted: Attested Xerox copy of the following Certificates
1. HSC, Certificate, +2 Science Certificate or equivalent
2. D- Pharma Passed certificate.
3. Registration Certificate of D- Pharma.
4. All Mark Sheet for verification.
5. Certificate of Cast and PH

TERMS AND CONDITIONS:
1. Candidates of Puri District can Only apply. In case of non-availability of candidates from Puri District, the candidates of other districts will be considered.
2. The Appointment / Engagement is purely temporary & on contractual basis and can be terminated at any time without assigning any reason thereof.
3. The selected candidates shall have to submit an undertaking to that effect that, she not claim any Government Post / Regular Appointment in future.
4. Age Limit: The candidates shall not be less than 18 years & but not more than 32 years as on dt. 12.06.2015. As per Orissa Civil Services (Fixation
of upper age limit) Rules will be applicable.
5. The number of Posts may deviate as per norms / present vacancies.
6. The ORV Act & Rules will be applicable for filling of the above vacancies.
7. The complete application with all enclosures should reach to the office of the Chief District Medical Officer, Puri At/ Po/ Dist- Puri (Odisha), Pin- 752001 super scribed “Application for engagement of Contractual DDC Pharmacist must be mentioned on the top of the envelope” and all application will be accepted only by the Regd. Post / Speed Post only. The last date of receipt of the application in the office of the CDMO, Puri is on or before 28.07.2015 up to 5.00 P.M. The Authority is not responsible for any postal delay/ missing of application.
8. Detail of documents to be submitted with the application form
(a) Valid Employment Registration Card.
(b) Caste Certificate for SC/ ST/ SEBC Candidates.
(c) Fresh Valid Residential / Nativity Certificate from concerned Tahasildar.
(d) Valid Physical Handicapped Certificates.
(e) All Educational Certificates.
9. The undersigned have all reserves of right for cancellation / modification of advertisement and mode of selection (as will be decided by the Selection Committee and as instructed by the Government).
10. Regarding amendment, addition, correction, alternation & others, it is the responsibility of the applicant to collect the information from NIC Website fwww.puri.nic.in') of Puri District.
11. As per the G.A Department Notification No. 32010 / Gen. Dt. 12.11.2013, the person appointed on contractual basis in the scheme and Programme in the Health and F.W Department who shall be less than 45 years of age and shall have completed at least one year of continues of service shall be allowed relaxation of upper age limit for entry into the Govt, service, provided that they satisfy all other eligibility criteria for the post as laid down in the relevant requirement rule and they shall be allowed one percent extra mark on the total mark of the examination for each completed year of continues service subject to maximum of fifteen percent which shall be added to the marks secured by them for deciding the merit position and the Xerox copy of contractual appointment order for scheme and programme and no objection certificate may be enclosed with the application form for verification.
