Qualification & Experience: Diploma in Pharmacy form a recognized Institute and with 3 years experience in the line in a large (more than 200 bedded) hospital.

Application form (Off Line) will be received by Registered/Speed post only. Applications by hand will not be accepted. Institute is not responsible for delays. No applications will entertain after 21-06-2013 by 5.00 P.M.

Application Fee: Application fee will be Rs. 500/- for General & OBC and Rs. 300/- for SC/ST. Application fee will be received through Bank Draft only and should be in favor of “Director SGPGIMS Lucknow”, payable at Lucknow. Application form without Application Fee will be rejected.

Last Date: Last date of receiving complete application form (On-Line/Off Line) along with fee and necessary enclosures is 21-06-2013 by 5.00 P.M.

Age: The upper age limit & experience will be reckoned as on 01-01-2013. The upper age limit is as follows:
Forty (40) years for General candidate and
Forty five (45) years for SC/ST/OBC. (Only for those having DOMICILE in the state of Uttar Pradesh ) candidate.

Reservation:
Reservation is applicable for SC, ST and OBC of Uttar Pradesh state only. SC/ST & OBC candidates of other states will be considered in general category.
Reservation norms for category (SC/ST/OBC) will be determined as per notification of UP GOVT. only.
Reservations for physically handicapped will be admissible as per U.P. govt. rules.

HOW TO APPLY:
Application form can be submitted through:
1. Plain paper on the prescribed format downloaded from the website sgpgi.ac.in/... From 28-05-2013 to 21-06-2013 Application fee will be Rs. 500/- for General & OBC and Rs. 300/- for SC/St, and posted by Registered/speed Post.
OR
2. Online applications at sgpgi.ac.in / …From 28-05-2013 to 21-06-2013 Application fee will be Rs. 500/- for General and OBC candidates and Rs. 300/- for SC/ST candidates.

After online submission, a printout of the filled-in application form (THREE pages) must be obtained and sent to :

The application form must be accompanied with the following essential relevant documents in support of: Application fee, age, qualification, experience, certificates in support of reservation (caste, physically handicapped etc).

General Instruction:

Envelope should be super scribed with the advertisement no. and name of the post.
Director reserves the right to cancel the advertisement in part or full and the selection process at any stage without assigning any reason and in that case application fee shall not be refunded.
Number of posts may be increased/decreased depending upon the need of the Institute.
No TA/DA shall be admissible for attending written exam/screening test/ interview.
Applicant working in Govt./Semi Govt./Undertaking/Autonomous institute should apply through proper channel or produce No Objection Certificate at the time of Interview/Test incomplete applications (i.e. without testimonials, document, bank draft, unsigned applications) and applications received in this Institute after the expiry of the last date will be summarily rejected.
Candidates with higher qualifications shall be preferred.
Self certified copy of testimonials/documents in support of age (date of birth) educational qualification, experience and caste must be enclosed with the application. Candidates are advised to write Registration No (written on right corner of application form) on each enclosure.
Merely fulfilling minimum requirement does not essentially qualify candidate to be called for test/interview.
Bank draft should be in favour of Director, SGPGIMS, Lucknow (UP) payable at the State Bank of India, SGPGIMS, Lucknow. Branch, Lucknow code no. 7789 must be enclosed along with the application which is non refundable.
Original documents would need to be verified at the time of interview/joining.
Question papers shall consist of multiple choice objective questions without negative marking.
Syllabus of written would be as per the eligibility criteria.
