Post: Patient Care Advisor

Job Description:
· Counsel patients for improved health related outcomes at the hospitals, clinics and home visits if required.
· Responsible for achieving set targets for patient enrollment
· Meet patients at regular intervals as advised
· Manage the compliance and adherence of patients enrolled.
· Collate health records related data from consenting patients
· Capture details related to Adverse Drug Reactions reported by patient in pre-defined templates
· Handling patient queries and providing response from pre-defined FAQs
· Provide patient support by coordinating with pharmacists, stockists and carrying & forwarding agents

Candidate Profile:
· Qualifications: B.Pharm / B.Sc (Microbiology, Biotechnology, Dietics, Nutrition) fresher - 2 years of experience.
· Target Orientation
· Empathy and willingness to listen and relate.
· Fluent in English, Hindi and any one regional language (Gujarati, Marathi, Bengali, Kannada, Tamil, Telugu and Malayalam, Assamese, Rajasthani)
· Computer skills - Knowledge of MS-Office
· Ability to work in a process driven/ structured environment
· Candidates with two Wheeler preferred.
