Job Description
The Pharmacist Level 4 serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
• Dispenses medications accurately and in a timely fashion.
• Resolves questions regarding formulary and non-formulary medications, following policies and procedures.
• Provides dosing and therapeutic recommendations that take patient age/kinetic based on literature.
•Intervenes and documents interventions to correct/ improve pharmacotherapy.
• Maintains pharmacological knowledge by reading professional publications, attending workshops, conferences and C.E. programs to maintain professional competency.
Candidate Profile
Bachelor Degree from an accredited College of Pharmacy (ACPE)
