Applications for the posts of Pharmacist cum Data Assistant and Staffers for the Dedicated Mobile Health Team will have to be made on a totally temporary basis for 11 months from the Contract Based.

Qualification : Diploma in Pharmacy from a university given by Indian law in Degree or Diploma in Pharmacy or its equivalent. Must be registered with Gujarat Pharmacy Council.The candidate should have commuter qualification.
Age limit : up to 40 years

1) HSC marksheet,
2) Final year marksheet of bachelor degree,
3) Degree certificate,
4) Attempt certificate,
5) Proof of age,
6) Registration of Gujarat Pharmacy Council.
7) Any one of PAN Card / Aadhar Card / Driving License / Election Card Identity Card
