Applications are invited for filling of vacancy/ empanelment of candidates for the following posts under Zilla Swasthya Samiti, Kalahandi for Mobile Health Units (MHU) on contractual basis for a period of 11 months with monthly remuneration as noted against each and subject to renewal as per society norms basing on the performance and subject to continuance of the programme.

Qualification: Candidate must have passed +2 Science Examination under Council of Higher Secondary Education, Odisha/Equivalent and Diploma in Pharmacy from any of the 3(Three) Medical College and Hospitals of the State/any other recognized private institutions duly approved by AICTE and examination conducted by the Odisha Pharmacy Board and he/she must have registered his or her name in Pharmacist Council in the State and have possessed valid registration certificates as on the date of advertisement.

Age: 21 yrs - 32 yrs as on 01.04.2016

Monthly Remuneration: 9,350 /- + PI + KBK allowance

General Information and Instructions
1. The above positions are purely temporary and co-terminus with the scheme. Canvassing in any form will render the candidate disqualified for the position.
2. Details of vacancy, eligibility, age, scheme of selection procedure etc. can be downloaded from the district website kalahandi.nic.in
3. Candidates are required to apply in prescribed format available in the website kalahandi.nic.in and to submit along with a set of self attested copies of all certificates in support of age, qualification, experience, residence etc. Candidates are also required to submit two recent passport size colour photograph, an ID proof (Voter ID/ PAN/ Driving Licence/ Passport/Adhar card) and recent Caste & Residence certificate issued by the competent authority. No certificate will be accepted at later stage of recruitment, if fails to produce with the application. Incomplete application in any form will be rejected
4. Candidates, who are already working in Health Department either on regular or on contractual basis, have to submit No Objection Certificate from concerned employer.
5. For all the post candidates must have passed Odia up to M.E Standard.
6. Interested candidates fulfilling the eligibility criteria are to apply to the "Chief District Medical Officer, Kalahandi, At/Po-Bhawanipatna, District-Kalahandi (Odisha) PIN-766001" so as to reach on or before 8-04-2016 through Regd. Post /Speed/Courier Post only and the envelope containing the application should be super scribed clearly name of the post applied for.
7. The applications received for all the posts will be scrutinized and the database will be uploaded in the above website for invitation of objection. Candidates are required to visit website: kalahandi.nic.in at regular intervals for any notification, updates (objection invitation/different tests notices), results etc. relating to recruitment.
8. If any candidate is found to have suppressed any material information or furnished false information/ documents, his/her case shall not be considered for the post applied for and in case already engaged on the basis of the said information /documents, his/her service shall be terminated from the Society forthwith. Candidates who have been disengaged earlier from the OSH&FW Society on administrative ground such as disobedience/poor performances/ misbehavior /criminal activity etc. are not eligible to apply.
